Between walls and networks: the threads interlaced in the hospital discharge process of critical patients

Abstract
This study aims to understand the discharge process of critical patients from the perspective of the social worker. Has a qualitative approach, using the critical dialectical method. Occur througt a semi-structured interview with family members of critically ill patients, happened between May and August of 2020, at Hospital das Clínicas in Passo Fundo/RS. The data shows the family as the place of care, but some of the family members cited gaps in this process, like the fragilities of promotion of patient care, both in the hospital and at home, which are interpreted as social determinants and conditions in the health-disease-process. Has as conclusion important elements to reflexion about discharge process of critical patients, seeking deppen the discussion and constructions of discharge preparation and the transition of care in the hospital environment.
